ALARM ACTIVATION REPORT

Instructions

There are two options for submitting Alarm Activation Reports to the SFMO.

(1) The first option, using the Hardcopy Alarm Activation Report below, allows departments to print the form, fill it out by hand, and fax the form to the designated SFMO office.

(2) The second option, using the Online Alarm Activation Report below, allows departments to fill out an Alarm Activation Report on this page and requires no printing or faxing of forms.

 

Option 1: Fax a Hardcopy Alarm Activation Report

Alarm Activation Report - Microsoft Word format - Download this form, complete it, and fax it to the designated SFMO office.

 

Option 2: Submit an Online Alarm Activation Report

Complete the form below and click the "Submit" button. Note: After you click the "Submit" button, a text file with the information you just submitted will appear in your browser. To save a copy of this text file for your records: (1) Click on the "File" button at the top of the page. (2) Click on "Save As" in the drop down box. (3) Type in a "File Name" for this report. (4) Click on the down arrow \/ in the "Save as type:" box. (5) Select "Text File (*.txt)" and (6) Click "Save."

             Negligently or accidentally activated alarm signal (DFIRS codes 741 - 746)

             Failure to notify fire dispatch center when testing the fire alarm signaling system (DFIRS                           codes 741, 742, 744)
             Fire alarm panel was reset prior to fire department arrival (DFIRS codes 741 - 746)
